Immunisation Record for pre-primary children
Kindly submit your child's record of vaccination & medical history also blood type for medical emergencies.
Birth Certificate
Please submit birth certificate of your child.
2 Passport Sized Photos
Please submit two passport size photos of your child.
School Leaving Certificate
If your child has moved from another school, then submit the school leaving certificate.
